Generated content, CR patch

Hi everyone

I've finally fixed the last regressions, handled the last corner cases and 
cleaned the code. So here is the latest implementation of generated content; 
please test and review it for immediate commit.

It closes wish #59507 and bug #96084, and as far as I know completes our 
implementation of CSS 2.1 for regular media.
