unexpected changes to konqueror right-click menu

Can kde by itself add icons to the panel and change the right click
menu in Konqueror? I  have seen both of these things happen in 
the last week. This morning right clicking on URLs in konqueror
brings up a menu with a new entry ("open in this window") above
what used to be the first entry in the menu ("open in new window").
The new menu entry disappeared when I restarted kde. Where is
this feature controlled?
