Works fine here. Hmm... did you try "bug:57590", may be you need a different

I think I had a similar problem the first time I tested my patch as well. The 
only thing that worked was to restart KDE after installing khtml. For some 
reason restarting kdeinit was not sufficient for me. And I do not even have a 
the "preload konqy" option set. I will give it a try.

Nope. 

BTW if you can see the above bug report, Stephan merged a another one into it. 
This new bug report is not about images, but rather links and other types of 
files such as PDF's. So restricting the solution to images might not solve 
the whole issue?? I did not realize that at the beginning either...
