[Kexi] Missing form designer

Isn't this "Createobject: Form..." ?
Is you package koffice-database-1.4.95-1.i586.rpm ?

Where have you installed your binaries from?
something like: 
ftp://ftp.kde.org/pub/kde/unstable/koffice-1.5-beta2/SuSE/ix86/10.0 ?

Note that you cannot have installed a different Kexi version from .rpm on the 
same system. 

Can you try earlier version ?
ftp://ftp.kde.org/pub/kde/unstable/koffice-1.5-beta1/SuSE/ix86/10.0/koffice-database-1.4.90-0.1.i586.rpm
