this should not be required. We enable c++11 globally. Do not depend on C++14.

maybe instead compile-in the debug files instead of linking against it?
refactoring/DocumentCache.h (line 42)

again: here and below: do not take by rvalue, just take a plain value
refactoring/DocumentCache.h (line 54)

why store it in pointers?
also: style, prefix members with `m_`, not just `_`.

this is the only place where you need the std::strings, right? Maybe use QByteArray before and only convert to std::string here on-demand. that simplifies the rest of the code thanks to implicit sharing, and you could simply use a QHash<QByteArray, QByteArray> for the map.
refactoring/utils.cpp (line 32)

here and below: style: put the { of a function on its own line
void foo
{
...
}
refactoring/utils.cpp (line 76)

add a Q_ASSERT(offset + length < text.size());
refactoring/utils.cpp (line 82)
<https://git.reviewboard.kde.org/r/123963/#comment55846>
style:
auto shift = [&text, &lastLine, &lastColumn, eolMarker]
(unsigned start, unsigned length)
{
....
};
refactoring/utils.cpp (line 121)

remove

they are not scary, but not used by us.

you are `using namespace KDevelop`, remove it here and below
