I still think the issue of duplicate commands should ultimately be fixed in a different way. However, after having looked especially at the breakpoint controller in more detail, I get the feeling that a proper cleanup is tough without binary compatibility breaking changes. So I think your approach is reasonable enough for the 4.7 branch. (Though note that I haven't done any testing at all...)
