Changes

Loads of changes in this patch. Basically this one is working as intended.

It now ports most (all?) features from oldpp. It will prioritise looking up in the duchain. If it cannot find any include from there, it will scan the include directories.
It now actually replaces the (equivalent, solution-less) Clang problem, giving less noise in the problem toolview.
