Cpp code completion bugfix and refactor

Attached is a patch with some bugfixes and a bit of refactoring for the 
cpp codecompletion's context.cpp, along with the requisite tests.

And a fair few unreported little issues (see new tests).

Doesn't change a whole lot in the way of functionality, but does make 
the context more consistent.

(I'd put it on the review board, but I'm not sure what account that 
requires)
