How to genarate .toc and .index files from doxygen documentation?

There used to be some utilties in the /doc directory for the generation of 
these files, as they pertain to KDevelop 2.x's Documentation Browser.  I 
think I cvs removed them a long time ago, but they should still be viewable 
through webcvs.
