If you don't feel confident that you know which bit of data is where,
in your csv file, open it in a spreadsheet first. Once you are happy
that you can sort out the order of columns in the csv file you'll find
it quite easy to match the data up with the kaddressbook fields. IIRC
I created a couple of miscellaneous-named fields to pick up data from
my old file where it was additional to the standard kaddressbook fields.
