 There was indeed a /home/david/Maildir/cur/cur:2,ST.  I have no idea how it 
got there.  I certainly didn't add it, and there's no way that David 
could/would have done.  It was empty, and removing it has solved the problem.

Interestingly someone else had had the same problem recently.   I suspect that 
dovecot has one of those bugs that's only triggered when venus aligns with 
mars, the user was born under pisces and the date is the 23rd :-D
