This box is only displayed for pure HTML messages. It is not displayed
for messages which include both an HTML version and a non-HTML (plain
text) version of the message text.
For the latter messages in KDE 3.5 one could click on the HTML part of
such a message in the "message structure viewer" (i.e. the thingy
usually shown below the preview pane unless it has been disabled in the
settings). Then the box was displayed and one could enable HTML for
this message.
I have no idea whether it does still work this way in KDE 4.3.
