I don't think this possibility exists in kmail fro kde3. However, I
dimly remember a discussion about such a labeling feature for future
versions of kmail on this mailing list. You may want to search the
list archive.

The old "Mark as To-do" doesn't do anything except marking the message
with a nice little icon. Recent versions of kmail (possibly the
enterprise branch of kde-pim which is included in many of the
mainstream distros) offer a "Create Task" option in the context menu.
It will create a to-do item (so much for intuitive naming of menu
items ;-) ).
