This is sheer guesswork. It must be seeing two entries, so -
Are these addresses in both addressbook and recent-addresses? Do you sort
your addressbook by 'lastname firstname'? If the recent-address list has the
name in the form you don't use in the addressbook, clean it out from
recent-addresses. Next time you use it, it will go in using the preferred
format.
