The setting appears to stick now.

I installed langpacks-en

I then went to System Settings -> Region -> Language and selected English as my
preferred language.

I then went to System Settings -> Region -> Spell Check and selected American
English as my standard language.

The setting in KMail's identity configuration now appears to stick.

However, automatic spell check is still not happening.
