I'm running Kmail 4.14.2 on Debian testing/unstable and I still have this
problem. I can't create top-level folders for IMAP resources. It can be
reproduced exactly as described in the bug report above, i.e.:

1. right click on the imap account name/folder and there will be no "Add
folder" entry.
2. Select an imap account name/folder and then click main menu "Folder": "Add
folder" is greyed out.

I work around this by creating folders in a roundcube installation that runs on
the server. For what it's worth, the IMAP server is Dovecot.
