I have just come across this problem (with Kmail 1.13.3).
I had defined a new 'Send' account, but not as Default.
However, the 'Special Transport' box for the Default account had been set as
the new account - but NOT BY ME!
Deselecting that box restored Kmail to using the Default account.
This is a nasty hole to fall into...
