Okay, whatever it was, it was either fixed, or the incidence rate seems to have
slowed down dramatically in kmail 4.4.5. I haven't noticed this in a while.
(Server still unchanged, kmail upgraded to kmail_4.4.5-1_i386 on debian
squeeze.)

I will continue to monitor this, and will report back if there are any more
duplicates created.
