I could clarify that problem in IRC #kontact.

The problem was caused by an underscore in the hostname which is in illegal
hostname character. Before it worked anyway, but a change in Qt has made
hostname parsing more restrictive and this seems to lead to the problem
described above (Kmail sends an EHLO without hostname).

Possible workarounds/solutions:
1.) Change your hostname to contain only valid characters.
2.) Override your hostname in KMail SMTP settings with a fake hostname that
contains only valid characters.
