I'm observing the same problem in KDE 4.3.5 (Kubuntu 9.10). I had changed the
default icon size (in KMail, starting Kontact to default to Summary view at
start) from "Default" to "Large" and back to "Default" (or "Medium") but it
sporadically forgets the setting and goes back to "Large".
