You were close... It is, in fact, in two places:
* the systemsettings, in the file associations.
You need to remember the file extension or the mime-type, search for it, and,
remove the extension and/or kwrite from the programs that handle that
mime-type. (or lower its priority)

* The do not ask again .... Is in the file
kmailrc[2]
In the section

[Notification Messages]
askSave_image/jpeg=2    (where jpeg is the mime type of the attached file)
(remove that line, and probably restart kmail)
