This works fine if you print in a .pdf as I described above:
''- Select a printer. Warning: this bug does not happen if you select a .pdf
file. Please use a real printer.''

The bug only occurs when using a printer. 

I retested this today using a different printer (inkjet instead of laser so
that it's a different printing driver) and I still get the same result: the
.pdf has headers but the printed Contacts have hearders only on the first page.
