Seeing the same behavior here with KMail 1.12.2 / KDE 4.3.1 from OpenSuSE 11.1. 

Swap "Sender" and "Subject" columns by dragging "Sender" to the left. Works
great, but after restarting KMail, the columns are back where they were.

Add new column. Drag it in to some other position. Restart KMail and it'll be
back over on the right.
