On Mandriva 2009.1 under KDE 4.2.2, and KMail 1.11.2 i have a similar problem.
This problem somehow may have something to do with Unicode and Locale also,
because when i changed name of one of the mail folder to Hindi name (though i
am using english locale for kde), and utf-8 charset in KMail, the problem went
away for that particular foler, but resurfaced when i made some other
configuration changes to kmail.

So for me, right now for some mail folder only Reply-to-All button is grayed
out while for some it is not.
