I've seen this with KMail 1.10.1 on KDE 4.1.2 and some KMail (don't remember
exact version) on KDE 4.2.  I use 'Send Later' as default sending method.  Menu
item is often disabled and the workaround is to go into outbox and select a
message, as written in previous comments.
