Hmmm, not sure what's up with the SVN->BKO link, but commits 1011062 for trunk
and 1011065 for 4.2 disable the From/To print range in the print dialog for
KHTML and consequently for KMail so users will no longer be confused by this. 
This is the best we can do for now I'm afraid.

I am working on patches to Qt to enable range printing via CUPS, but that will
not be available until Qt4.6 (fingers crossed).

I'm not sure khtml would ever be able to be modified to directly support page
selection, but that would be a separate wish request.
