I've tried to reproduce this bug killing my running gpg-agent, then send an
encrypted mail, and kmail sent an encrypted mail without asking the password.

Do you have runing processes like?
/usr/bin/gpg-agent --sh --daemon --write-env-file /home/user/.gnupg/agent.info
/usr/bin/ssh-agent /bin/bash /etc/X11/xinit/xinitrc
kgpg -session 10d3d8da65000122107096600000039780007_1238527063_33663
