QtCurve - but it doesn't offer a KWin theme (yet) under KF5.

It's highly configurable so don't let its default look put you off.  There's also no official release (yet) so distributions don't always ship it, or an older version (which I guess shouldn't be a problem for a Gentoo-testing user ;) )
