Konsole menu bar disappeared

For some reason the menu bar in Konsole disappeared. I have tried:

1. Right-click in Konsole and make sure "Show Menu bar" is checked. I 
also tried switching it on and off a few times.

2. Bring up the Konsole settings and make sure "Show Menu bar" is 
checked, and click apply. Again, try switching it on and off a few times.

3. Reinstall Konsole.

Any ideas?
