KDEinit could not find/launch

I've just been asked about an error message "KDEinit could not find firefox 
executable".  This reminded me that I've fairly frequently seen "KDEinit could 
not launch kwrite" - even though often it has launched anyway.

Has anyone any idea what goes wrong?
