kMail how to increase the left margin?

Assuming you have kprinter and LPRng, do the following:

Click on File/Print

At the top there is a box with the title 'Printer'.  Just below is a line with 
'Name' To the right is a button with the label 'Properties'

Click on 'Properties'. Another box will appear with a tab marked 'Margins',

Click on this tab, select 'custom margins' and save.
