I think it is created from the files in the applnk directory in your 
global KDE directory.
Here, on my system, KDE is installed in /usr, so the applnk directory is 
/usr/share/applnk

Just add a .desktop file for the new program.

Usually distributors have a /etc/skel directory which gets copied into 
the newly created user's home directory.

If you have a .kde or .kde2 or .kde3 (.kde ist default, others can be 
overrides if $KDEHOME is set) in there, it will get copied into the new 
users's home dir too.

It can also contains a share/applnk directory, which will be merged with 
the global stuff.
