You can do anything Qt can do without a visual tool like Designer.
Designer helps you in doing the visual parts, so you get a better idea
how it will look like, how the layouting is, etc.
So you can create the widgets with designer and then implement the
functionality with KDevelop.
The usual first steps are:
1) create project in KDevelop with approrpiate type (Qt app, KDE app, etc)
2) create dialogs, widgets in Designer
3) use those dialogs and widgets as the base of your UI in KDevelop.
