What the buttons should do is based on the location of the buttons.
Where the buttons should be placed is the question you want to ask here.
Let me explain:
the singlePanel.png attachment shows the buttons inside the panel and the 
wholeDiag.png shows the buttons outside the panel.

Thus the buttons define which part of the dialog to act on. Since the defaults 
and the apply buttons are both either located inside or both outside of the 
panel their actions are always happening on the same content.

KDE choose the wholeDialog style of committing for all single applications 
config dialogs. The attached one is from konqueror. And I feel KOffice should 
simply use that dialog type.
