Indexes in documentation

Yeah, that's what we're using - it still requires you to define the places the 
index will point to (using <indexterm>s). It's adding those <indexterm>s that 
I thought might be a task suitable for new contributors. Sorry if I wasn't 
too clear on that the first time.
