I no longer have information for previous versions of KDE/Qt/CUPS.  I was not
aware the dialog was from Qt, in which case it appears to be a regression in Qt
and not KDE.  

I have CUPS 1.4.6 and Qt 4.7.3 installed.  Looking at the changes to ports, in
reverse order, qt was updated from 4.7.2, cups from 1.4.5, qt from 4.7.1 and
cups from 1.4.4.  I will downgrade to those versions and report back my
findings.
