Thomas: Thanks for the answer. When describing the above case, this is
precisely what I did (hey, I *even used Kmail*!). Picked "Custom" as the paper
size, but the dimension boxes were all greyed out. This was under two different
printers (HP DeskJet 840c and HP LaserJet P1006).

I know this is part of Qt, not KDE; what I am trying to imply is: the
functionality previously present in KDE SC of the past is still not present. No
matter what the supporting architecture might be, it is still a regression to
the eyes of Joe User.
