You may want to export KSMTP_SESSION_LOG="some/file" and restart
Akonadi. This will help in seeing where it fails, if it fails somewhere
(BEWARE: it will contain passwords and what not, as it's the full log
of the SMTP conversation).

An alternative would also be to attach gdb to it and see where it's
stuck.
