I *just* ran into this myself. If I can reproduce it, I'll provide this info.

The debug from the akonadi server and agent was really nothing special: 
basically jobs were started and never got anything back. Just .. silence. :) I 
tracked that via the jobs tab in akonadiconsole.

The debugging tab in akonadiconsole also didn't say much. I could see folder 
listings (at least those that were working) with the headers for the emails 
being correctly sent .. but that was it.

So if/when I reproduce, I'll try and catch a backtrace of the maildir agent 
for you...
