I might want to add to this observation, this is not limited to kmail 
filtering. E.g. this also happens to me with a gmail account, and several 
"labels" (folders) in it, so filtered on the server instantly. In Gmail it is 
called archiving, so no copy is left in the INBOX, but only moved to the 
corresponding label folder. The system tray icon updates correctly, the 
folderview shows unread mails as well, but the message list doesn't get 
updated. 
Also, filtering the message list in kmail to show only unread mails doesn't 
help. Either going offline and online again or simply restarting kmail helps 
then.
Like the others, i don't see any pattern. Most of the time it works. Sometimes 
not. And it is also not related to the depth of the label folders.
Though, thinking about it, it never happened in the INBOX directly. At least 
not for me.
