Actually I think there is another tool called kmailservice (meaning KDE Mail 
Service, not KMail service) which forwards the mail as encoded in a mailto URI 
to the "default mail program" configured for KDE.
Maybe ksendemail does the something similar internally?
(KToolInvokation::invokeMailer() probably?)
