Unless I missed something all functional changes should be committed by now. Many of them turned out to be workarounds for bugs in the current code rather than PostgreSQL specific changes, so the actual implementation differes a bit from your patch. Therefore it would be nice if you could re-test with the current version, at least the database creation and some unittests seem to work here. Also, please mark thi review request as submitted.
