[Kde-pim] [Patch] alter table add index switch to create index

Hello everyone,

here is my first patch ever - so please be clement. :)

This patch switch from "alter table ... add index" syntax to the "create 
index .. on ..." syntax which is available in MySQL and PG. So for later 
support of PG.
