I think one step could be to check whether the problem is the sending or the 
composing of the message.

For that one could create a message with an attachemt and then save it as a 
draft. It is then possible to look at the mail's source, e.g. if it contains 
the attachment multipart, and/or save it so it can be examined elsewhere, 
probably even be sent by a different KMail.
