sysutils/k3b-kde4 update to 2.0.3

This is an overdue update of K3B to 2.0.3, which was released more than half a
year ago.

Almost half the patches can be dropped with this update. Many had come from
upstream commits between this release and the prior, and one of ours has made
its way upstream. Those patches that remain were refreshed with makepatch. One
new patch for current FFmpeg support was obtained from Gentoo. Updated
pkg-plist.
