Well, I had two separate problems, and one of them was my own fault.  I  
was using <ulink> instead of <link> so it's not surprising it couldn't  
find a url that was really just the id of the sect I really wanted to  
link to.

However, I think the missing section headings may be related to the  
missing prev/next links in http://bugs.kde.org/show_bug.cgi?id=271331,  
as I see the problem with docbook-xsl 1.76.1 but not on a different box  
with docbook-xsl 1.75.2.
