Just some guesses, until someone who knows more than me can suggest something:
Do you have the right public identifier, ie does the document start with:
<!DOCTYPE book PUBLIC "-//KDE//DTD DocBook XML V4.2-Based Variant V1.1//EN"
"dtd/kdex.dtd" [
Anything else is likely to cause problems (although it looks like meinproc is
finding the right dtd).
Perhaps different bits of kdelibs/kdoctools ($KDEDIR/share/apps/ksgmltools2 in
the installation tree) are out of sync. If you're building from cvs, try
updating that kdelibs directory and reinstalling.
You might want to send the file along to the list, to see if anyone else can
process it (then we know whether it's a configuration problem or a problem
with the document).
Hope some of that uninformed speculation helps :-)
