Bob, it's a *template* using fake names, and placeholders, for you to put real 
text in.

There is no application called KApp in KDE, so there is no definition for the 
&kapp; entity.  

When you write your own documentation, you will not use &kapp; you will use 
&k-my-own-application; and you will have to define what 
&k-my-own-application; is to expand to.
