I finally found /usr/kde/3/share/apps/ksgmltools2/customization/dtd/kdex.dtd
and I got http://ftp.gnome.org/pub/GNOME/sources/libxml2/2.5/

I think I've pieced things together correctly.
I got rid of all the failed to load errors (but I'm using docbook-4.1.2 
:-().

This is the first error I get now:

# checkXML template.docbook
template.docbook:47: error: Entity 'kapp' not defined
<title>The &kapp; Handbook</title>

Any suggestions as to why?
