I would use the alternate stylesheet and either print that, or create a pdf 
with html2pdf:

'meinproc --stylesheet 
$KDEDIR/share/apps/ksgmltools2/customization/kde-nochunk.xsl index.docbook'

There is a README in that folder, detailing the alternate stylesheets, and 
their intended uses.
